About the Role
The organisation is seeking a Health Improvement Practitioner to join their general practice team. The role involves delivering brief, consultation-based services to practice team members and patients, with a focus on evidence-based behavioural interventions for both mental and physical health conditions. The practitioner will support an integrated 'Model of Care,' facilitating smooth transitions between general practice, community-based NGOs, and specialised services. Empathy, care, and a nurturing approach are crucial in this role.

Key Responsibilities
- Provide brief, evidence-based behavioural interventions in individual, group, and whānau settings
- Collaborate with the GP team to deliver consultation-based services
- Guide clients through available services to support their overall well-being
- Engage in continuous professional development
About You
To be considered, you MUST have:
- Registration with a governing body such as the Social Worker Registration Board, Occupational Therapy Board, or New Zealand Association of Counsellors
- A current practicing certificate as a registered health professional
- Clinical experience in mental health, with skills in delivering psychological interventions
- Understanding of behavioural medicine and the connection between medical and psychological systems
- Strong relationship management abilities and familiarity with the healthcare system
- Excellent communication skills and the ability to work with diverse populations
- Strong organisational skills and a proactive approach to planning
