About the Role
As a HITH CCP clinician you will provide phone based assessment and regular clinical monitoring of high-risk covid positive clients that are admitted to HITH and are isolating in the community. Your assessment will include past medical history, comorbidities, current symptoms, psychosocial issues and ability to isolate safely.

  • Receive and process referrals from the NEPHU
  • Undertake initial clinical and social assessment of the client
  • Triage clients to appropriate level of care
  • Provide high quality and evidence based clinical advice and support to the clients
  • Provide regular phone based clinical follow up of clients with a focus on their symptoms of Covid 19 and isolation requirements

Classifications for this position include the following:

  • Registered Nurse Grade 3B Year 1 - Year 2 YU15 - YU16 ($96,408 per annum)
  • Occupational Therapist ​Grade 2 VF6 - VF9 ($83,361 - $ 95,825 per annum)
  • Social Worker Grade 2 SC21 - SC24 ($83,361 - $ 95,825 per annum)
  • Physiotherapist Grade 2 VB1 - VB4 ($83,361 - $ 95,825 per annum)

Your Contribution

  • Current nursing or allied health registration with AHPRA
  • Advanced assessment and clinical management skills
  • Ability to perform clinical assessments and provide clinical advice
  • Effective communication skills (including verbal, written and skills in adapting communication styles to suit patients, families and/or carers, other service providers and colleagues).
  • Current Police Check, or willing to obtain

About HITH Covid Positive Pathways (HITH CPP)

This position is part of St Vincent's COVID response. The intent of HITH CPP is to support high-risk covid positive patients, admitted to HITH to isolate safely at home through phone based monitoring of their health, wellbeing and ability to isolate. These patients may be referred as a step-down from inpatient services, ongoing monitoring from ED, clinical oversight of patients from Sotrovimab Clinic or escalation of care from HealthMonitor. HITH CPP also ensures that those patients who are at risk of deteriorating are identified early and monitored closely to ensure rapid escalation where required.

This position is based at the St Vincent’s’ Hospital Melbourne Fitzroy site. Work in this program can be undertaken on site or remotely from the clinician’s home.
