A multi-faceted role within a large rural Community Health Service based in Limestone Coast, this is a great opportunity to join a positive and supportive working environment. As the Allied Health Clinician, Community Based Support, you will be accountable to the relevant Team Leader / Director for active participation in the country wide Transition Care Program. The core focus of your role will include identifying TCP recipients’ needs, planning, implementing, coordinating and overseeing appropriate service delivery options. You will work collaboratively with Referrers and Service Providers to plan and coordinate services appropriate to the needs of recipients, and ensure TCP reporting, accountability requirements and deadlines are met. Furthermore, you will assist in the supervision, support and development of health professionals / workers involved in the delivery of TCP services.

You must hold a recognised qualification within Occupational Therapy or Physiotherapy with eligibility for practising membership of the relevant professional organisation. A Masters with a related allied health undergraduate degree or appropriate post-graduate experience is also required. Bringing a strong background in community health, you will have the ability to apply professional expertise, generalist / specialist knowledge, clinical competence and experience to provide professional services to TCP clients requiring complex practice skills. A first class professional, you will be able to work under reduced clinical supervision, and proactively draw on the support of experienced peers when required. In addition, you will bring the ability to manage and prioritise personal workload and support others in developing workload management plans, including in the allocation of team resources.
