This is a great opportunity for experienced RN or EN's to join this large modern and innovative primary health care practice. The practice was voted Victoria's 2018 Practice of the Year by the RACGP.  You will work as part of a team with GPs, nurses and allied health professionals to deliver quality primary care nursing with a multidisciplinary approach. You will also support all areas of primary care nursing including triage, procedures and chronic disease management. There is the opportunity for nurse-led clinics from time to time. Patient demographics are typical for rural community with high level on mental health, chronic health , elderly and young families.

As part of a supportive team, you will have access to a shared learning calendar with internal and external presentation providers.  A comprehensive orientation and induction program will also be offered to you along with flexible working hours and negotiated salary above award - depending on skills and experience.

Essential Criteria:
  • Tertiary qualifications/Division 1 or Division 2 and experience in primary health care sector
  • Proven ability to work as a highly effective member of a team, including being part of a diverse group of allied health professionals
  • Ability to provide clinical leadership, and support to medical students, nursing students and GP registrars
  • Ability to lead or willingness to support innovative practices
  • Sound knowledge of the general practice environment, and its relationship to the broader health sector and local community
  • Extensive experience and knowledge in general primary care nursing and the day-to-day operations, or demonstrated capacity to acquire this experience
  • Demonstrated patient- focussed approach to service provision
  • Outstanding interpersonal, communication and teamwork skills - including written, verbal and negotiation skills
  • Demonstrated ability to provide support and effective communication within the practice and with external stakeholders
  • A commitment to and experience in a continuous quality improvement to maintain accreditation
  • A commitment to ongoing professional development.  It is a requirement to participate in relevant CPD activities consistent with National Registration requirements including annual CPR
Highly desirable:
  • Understanding of the Medicare Benefits Schedule - especially in relation to Chronic Disease and Primary Care Nurse item numbers
  • Experience with Team Care Arrangements, Coordination of wellness clinics/chronic disease management and knowledge of local community resources
  • A qualified Nurse Immuniser with current credentialing in nurse immunisation or working towards credentialing
  • Asthma educator with knowledge of spirometry & Immunisation
Additional requirements:
  • Availability for weekend and evening work,
  • Current Victorian Driver’s License and access to a private vehicle
  • Satisfactory police records check

Daylesford is just over an hour from Melbourne. It's twin town of Hepburn Springs is located in a region boasting the largest concentration of natural mineral springs in Australia.

The town attracts a thriving artistic community, with beautiful mountain scenery, forests, gardens, lakes, and locally grown gourmet produce has seen the town develop as a major holiday spot and alternative lifestyle town. Nearby Ballarat also offers great cultural opportunities and first-class shopping.
