Occupational Therapist- Adults Grade 2 : Broadford

With a 40 year history, Nexus Primary Health (Nexus) pride themselves on being the "go to" organisation for primary health information and person-centred services, with offices located in Broadford, Wallan, Seymour and Kinglake and across the Mitchell, Murrindindi, Greater Shepparton and Strathbogie Shires.

Working from their newly refurbished Broadford Centre, Nexus community health have an opportunity for an experienced Occupational Therapist to join their dynamic friendly team.

The Occupational Therapist is a member of the Adult Allied Health Team which includes a full multi-disciplinary team who provide services under NDIS, Commonwealth Home Support Program, Home Care Packages, Home & Community Care – Program for Younger Persons and self-funded. The Adult Allied Health Team and other Nexus Teams aim to work together to provide co-ordinated, multidisciplinary care.

You fulfill a diverse and flexible role in the provision occupational therapy assessment and intervention to adults with a broad range of disabilities, particularly to those who have an NDIS Plan; The role is primarily based at Broadford with some visits to other Nexus sites and outreach work when required. You will work with clients and relevant others to create, implement and review a holistic Care Plan that reflects client goals and help develop strategies to optimize functional capacity and to be creative and flexible with approaches and strategies.

As well as 1:1 clinic based support services or home visits where appropriate , you will also conduct group therapy programs for suitable clients and families and service other funding streams such as CHSP, HACC-PYP, HCP and self- funded clients as required.

As a Grade 2 you will also have opportunity to supervise and support Grade 1 OTs, OT students, Allied Health Assistants and other students undertaking placements.  There is also Health education/Promotion responsibilities and you will play a role in the planning, implementation and evaluation processes of program development and projects.

Key Selection Criteria: (Please address this in your cover letter)
  • Bachelor of Occupational Therapy with AHPRA registration.
  • Minimum five years’ experience as an Occupational Therapist.
  • Minimum two years’ experience working with clients with NDIS Plans
  • Well-developed Occupational Therapy knowledge base and skill level, particularly in the area of NDIS and disability, including the prescription of equipment and home modifications.
  • A philosophy of empowerment and person-centred care.
  • A commitment to further developing knowledge and skills.
  • Excellent organizational and time management skills
  • Well-developed interpersonal and communication skills, including the ability to initiate and maintain contacts with a broad range of service providers
  • Ability to work independently and as part of a team.
  • Current First Aid and CPR qualifications.
  • Driver’s Licence
