About the Department

The Access and Demand Unit provides organisational support for patient access and flow for timely access to care across 7 days a week. We provide a supportive environment for all members of our team, providing orientation and ongoing development support.



About the Role
The Patient Flow Coordinator (PFC) supports the Access and Demand Manager (ADM), to facilitate patient access and patient flow. The PFC plays a pivotal role as part of a centralised bed management system in monitoring the bed occupancy levels and the demand for inpatient beds.

The PFC is responsible for consistent and timely bed allocation aligned to the organisational values and imperatives. The PFC are a point of contact and co-ordination for external enquiries regarding inter-hospital transfers and direct admissions from the community.

The classification for this position will range between a YW17-YW18 ($101,764 base salary per annum - pro rata for part timers).



Duties

  • Ensure that systems and processes are in place to coordinate safe and timely access of patient care
  • Using critical thinking skills to allocate and coordinate patient flow
  • Assessment of whole of hospital approaches to problem solving
  • Apply clinical assessment skills to prioritise and critically evaluate coordination of patient flow
  • Point of contact for internal and external requests for access and demand related to patient flow

Skills, Qualifications and Experience

  • Experienced nursing leader with exceptional communication skills Clinical expertise working or working towards ANUM level
  • Past experience of bed management in Access and Demand
  • Able to demonstrate experience in working with multidisciplinary teams in complex health care environments
