The successful candidate will work as an interdisciplinary team member to provide comprehensive, integrated and high-quality physiotherapy services to community clients attending services such as residential aged care and commonwealth support programs. The position is primarily based in Port Fairy with the opportunity to work off-site for home-based clients.

As part of the role, the successful candidate will conduct comprehensive assessments, formulate intervention plans and professional recommendations in patients' medical record according to the hospital and professional standards.

The successful candidate will also provide professional and clinical leadership to staff, patients and carers on matters relating to Physiotherapy. They will also be required to communicate effectively with external and internal stakeholder groups.

The successful candidate will have access to an annual in-house education calendar and will undertake ongoing professional development sessions.

Essential Criteria:
  • Current registration as a physiotherapist with the Australian Health Practitioner Regulatory Agency (AHPRA).
  • Demonstrated understanding of, and commitment to, the role of the physiotherapist in community and residential aged care settings utilising a person - centred approach.
  • Demonstrated knowledge of program funding and referral pathways.
  • Demonstrated experience within a rehabilitation/restorative care program with a focus on wellness and reablement.
  • Including implementation of comprehensive, goal directed care plans.
    Demonstrated ability to be self-directed, motivated and to manage time effectively.
  • Working knowledge of electronic data bases, recording statistics for compliance with funding targets.
  • Demonstrated commitment to ongoing professional development and ability to meet the requirements of the Moyne Health Allied Health Credentialing and Scope of Practice.
Desirable Selection Criteria:
  • Knowledge and understanding of The National Safety and Quality Healthcare Standards and Aged Care Accreditation Standards.
  • Intermediate skills in Microsoft Office programs.
  • Knowledge of TRAK, RISKMAN, Platinum.

The organisation is a public, not for profit, integrated healthcare service that helps meet community healthcare needs in Port Fairy and surrounding districts in the Moyne Shire.

As a rural health service, it provides a range of integrated, person-centred care that helps meet community needs. Based in Western Victoria, it provides a wide range of three core services including acute hospital care, aged care and community health. The team is a supportive and dynamic multidisciplinary group of professionals committed to their community.
