This role will allow you to provide physiotherapy services in a multi-disciplinary setting to deliver services across the local, aboriginal and outreach community, as a part of Mobile Health Hub.  Working with integrated multi-disciplinary team, incorporating GP's, Allied Health, District Nursing and Home Care Packages.

Essential Criteria:
  • Experience working within a community health care setting and / or within a multidisciplinary team.
  • Demonstrated ability to work professionally, both independently and collaboratively within a team environment, to achieve desired outcomes.
  • A commitment to provide professional physiotherapy services, incorporating evidenced based practice to guide interventions. An understanding of The Wagner Chronic Care Model is desirable.
  • Demonstrated ability to identify and create opportunities to enhance care through investigative literature research, with a focus on continuous quality improvement.
  • Proficient time management and prioritisation skills.
  • Demonstrated ability to achieve statistical targets within an allocated timeframe, and maintain accurate medical records.
  • Professional interpersonal, oral and written communication skills, including proficiency in the use of clinical information systems, MS office, internet, and other computer software packages.
  • Post graduate qualification in  Physiotherapy
  • Current registration as a Physiotherapist with AHPRA
  • Current Police Record Check and Working with children check
  • Current Victorian drivers licence
Practice Details:
Working for this small rural hospital and community health centre located in Horsham on the Fitzroy River that provides urgent care, acute and residential aged care and a range of primary care and allied care services to the local farming community and surrounding rural towns.
