How you will make an impact
As a Registered Nurse, you will play a vital role in assessing, planning, and implementing services that address patients' health needs. You will primarily provide outreach nursing, focusing on wound care for patients in their own homes.

In this position, you will serve as a key resource for Cabrini’s Ambulatory Services, collaborating with both internal and external healthcare providers to coordinate patient care and facilitate access to Cabrini's Subacute Services, all within the framework of Cabrini Health's Mission, Values, and Vision.

Cabrini’s Hospital in the Home Unit is currently offering a permanent part-time opportunity for a skilled registered nurse with a passion for home-based care. This rewarding role allows you to deliver acute support to patients in their own environments.

Key responsibilities include:

  • Develop and implement comprehensive care plans for patients in their homes.
  • Consult with patients, families, caregivers, medical staff, and other healthcare team members regarding current medical needs.
  • Monitor and report any changes in patient status to relevant personnel to ensure timely and effective treatment.
  • Collaborate with all departments to uphold best practices for home patient care.
  • Work both autonomously and as part of a team.


You will have:

  • Current registration as a Registered Nurse with AHPRA.
  • A valid driver’s licence
  • A Current Working with Children Check
  • Proven experience working within the acute and community nursing


Key Capabilities:

  • In-depth knowledge of wound care and VAC therapy.
  • Competency in intravenous catheter cannulation.
  • Experience managing venous access devices and administering medications.
  • Chemotherapy experience (desirable but not essential).
