Senior Care Coordinator, Better Health and Housing Program

About the Role

The Sumner House site in Fitzroy has been used to support homeless people throughout the COVID-19 pandemic. All services at the site are delivered through a partnership between Brotherhood of St Laurence, Launch Housing and St Vincent’s, along with relevant government departments. The site reopened in response to Melbourne’s third wave of COVID in September 2021 to deliver the following functions:

  • The COVID Isolation and Recovery Facilities (CIRF), provides a safe and supportive environment for homeless people who are required to isolate due to being a primary close contact or due to being symptomatic for COVID-19.
  • The Better Health and Housing Program (BHHP) will support individuals with health issues who are experiencing chronic homelessness by providing an integrated health and homelessness service response for stays of usually 12-26 weeks. BHHP environment will support residents to rest and recover, and address health and social issues, according to the person’s own goals.


Classifications for this position will range from:

  • Physiotherapist, Grade 3 VB7 - AH22 ($96,205 - $113,604 per annum)
  • Occupational Therapist, Grade 3 VG3 - AH18 ($96,205 - $113,604 per annum)
  • Social Worker, Grade 3 SC31 - AH27 ($96,205 - $113,604 per annum)
  • Registered Nurse 4B YX11 - YX12 ($110,333 per annum)
  • Registered Psychiatric Nurse, Grade 4 NP74 - NP77 ($98,800 - $109,200 per annum)



Your Contribution

This position provides care coordination to residents in the BHHP. The senior care coordinator role is an experienced care coordinator who will be:

  • Working within a client-centred framework to complete comprehensive holistic health assessments and/or discipline specific assessments,
  • Supporting residents to develop and work towards goals of their stay at BHHP.
  • Providing specific expertise in mental health and/or alcohol and other drug use.
  • Registered Nurse, Registered Psychiatric Nurse or Allied Health 7 years’ experience in acute/community setting.
  • Eligible for registration/membership with AHPRA
  • Current Victorian Driver’s Licence
