There are almost half a million Australians affected by chronic or slow to heal wounds. With an ageing population, it is estimated these numbers will rise. Although wound management principles are touched on briefly in undergraduate nursing education programs, often only the basics are covered. In addition, new wound dressings are advancing and becoming more complex.

A comprehensive assessment of the person and their wound is vital to direct person-centred treatment. An understanding of common wound types and their management which are seen in everyday practice are also important. Nurses must stay abreast of the latest knowledge, tools and wound management therapies to facilitate evidence-based person-centred practice. This comprehensive study day will provide clarity and useful information to incorporate and succeed in the person-centred wound management delivered in the busy modern nurses’ practice.

This workshop will incorporate the principles of wound management for the following wound types:

• Skin tears
• Pressure injuries
• Malignant wounds
• Surgical wounds
• Leg ulcers – venous, arterial, mixed
• Diabetic related foot ulcers
• Burns

Learning Outcomes

Skin Anatomy, Physiology & Wound Prevention   

• Identify the anatomy of the skin & associated structures
• State the main functions of skin & its importance in the body's defence mechanisms
• Identify health promotion & evidence-based wound prevention strategies
• Display an understanding of the physiology of normal wound repair & the body's response
• Identify common wound types found in the person with multiple co-morbidities

Wound Management

• Recognise physiological differences between acute & chronic wounds & demonstrate knowledge of common wound types
• Identify factors that affect wound healing & strategies to minimise their impact
• Recognise further investigations that may be indicated
• Identify when referral to specialist services are indicated

The Importance of Assessment

• Explain the importance & process of undertaking a person-centred holistic assessment in Wound Management
• Undertake assessment of the clinical characteristics of the wound bed & surrounding skin in consideration of the T.I.M.E principles e.g. dimensions; grading; exudate-amount; type & colour, odour; wound edge appearance; wound-related pain
• Recognise wound bed tissue types, exudate & wound edge issues
• Identify signs & symptoms of local & spreading wound infection
• Understand how biofilms impact on wound healing and the importance of mechanical wound debridement 

Wound Dressing Selection

• Understand basic dressing types & functionality
• Recognise key dressing types & when to use in practice
• Apply the principles of wound bed preparation in dressing selection e.g. supporting autolysis; managing infection & inflammation, moisture balance & addressing wound edge status
• Discuss care of the peri-wound skin & prevention of contact dermatitis, skin stripping due to adhesives & other complications
• Set local wound goals e.g. to protect, maintain moisture balance, aid autolytic debridement when clinically indicated
• Discuss risk assessment for aseptic technique type (standard versus surgical) as part of infection prevention & control applied to their local care delivery setting
• Recognise when the use of advance wound therapies is required
