AOD Care Coordinator - Primary Health Nurse

About the Role

The primary objective of the C No More Project Primary Health Community Nurse position is to pilot a 12 month mobile outreach care coordination service for people in Community Corrections or otherwise involved in the criminal justice system.

People on community corrections orders share similar risk factors for poor health as the prison population but may not access primary health care in the community. This population has not previously been targeted for primary health care in Victoria. The Gastroenterology Department received non-recurrent funding to implement a pilot mobile nurse and peer-led mobile model of care to offer rapid hepatitis C testing and treatment for people attending community corrections (August 2023-March 2025). The pilot resulted in 60 people diagnosed with hepatitis C, with 92% treated. Phase 2 (C No Moe PLUS) extends the successful model of care to expand our outreach sites, continue hepatitis C care and introduce vaccinations against hepatitis B and influenza, asymptomatic testing and treatment for sexually transmitted infections, and care coordination for co-occurring health and social disadvantages, including alcohol and other drug treatment, housing support, mental health support, where indicated and requested.

The overall aim of the C No More project is to increase uptake of hepatitis C treatment among hepatitis C positive people who have a history of criminal justice system involvement at community corrections sites through proactive engagement and follow-up. Our pilot identified that client’s social and health circumstances impacted their ability to commence and continue treatment for hepatitis C. C No More PLUS aims to provide further support for this population to engage in health and social care.

The C No More outreach team currently consists of two hepatology clinical nurse consultants and one peer worker (from Harm Reduction Victoria). Working closely with the C No More outreach team, this position will be responsible for:

  • Delivery of acute primary nursing assessment and care where indicated
  • Facilitation of referrals and support for people to access additional health services as required,
  • Development of pathways to streamline service access,
  • Undertake further scoping of the acute and chronic health needs of those attending the C No More van
  • Work collaboratively with SVHM departments including the primary health hub mental health clinician and Addictions Medicine, among others
  • Project implementation, including the development and maintenance of stakeholder relationships, client management processes, operational logistics and maintenance of equipment and resources,
  • Clinical data collection and maintenance in line with St Vincent’s Hospital regulations, and
  • Project evaluation data collection, maintenance and security in line with the project protocol.

Classifications for this position will Registered Nurse Grade 4B.

Your Contribution

  • Registered nurse (Division 1) with appropriate AHPRA (Australian Health Practitioner Regulation Agency) Registered Nurse Practising Certificate
  • Current Victorian Driver’s Licence
  • Completion of basic life support training/CPR/First Aid

About ALERT

This position will primarily sit within the SVHM Healthcare for the Homeless program multi-disciplinary ALERT Psychosocial stream and will provide an outreach service with the Gastroenterology Department’s C No More team to various street-based locations adjacent to community corrections sites in the Melbourne metropolitan region.

Since 2015, the Gastroenterology Department has provided a comprehensive state-wide program for the assessment and management of viral hepatitis B and C infection in the Victorian custodial system. Complementary fixed-term funding is supporting an initiative to link people on community-based corrections orders with blood-borne virus and sexually transmitted infections care and care coordination including linkage to alcohol and other drug services, housing and psychosocial support. The initiative is a collaboration between St Vincent’s Hospital Melbourne, the Burnet Institute and Harm Reduction Victoria and the project team consists of staff from each of these organisations.
