Care Manager - Home Care

Location: Sydney
Job Type: Full-time (Monday to Friday)
Salary: $80,000 + Superannuation + Commission Structure

We are seeking an experienced Home Care Manager to lead our dedicated team in providing exceptional care to clients in Sydney and we also have a postion available in Brisbane. This managerial role offers the perfect opportunity for someone with a strong background in case management and home care to make a meaningful impact in the lives of our clients.

Key Responsibilities:

• Manage a caseload of up to 75 clients, ensuring the highest standards of care.
• Conduct weekly client visits to assess needs and monitor care plans.
• Oversee the administration of home care packages, ensuring compliance and quality service delivery.
• Collaborate with care partners and support staff to develop tailored care plans.
• Implement best practices and policies to enhance client satisfaction and operational efficiency.
• Provide support and mentorship to team members in the field.

Qualifications:

• Proven experience in case management or as a care partner within the home care industry.
• Strong knowledge of home care packages and relevant regulations.
• Previous aged care experience preferred.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.
