Meg’s Care Team provides highly personalised, person-centred support to Meg, a young woman with complex physical disability requiring 24-hour care and support.
The team operates within a self-managed NDIS environment and is committed to providing consistent,
skilled and compassionate support that enables Meg to live a meaningful, connected and fulfilling life.

Position Overview
The Care Team Manager is a hybrid leadership, coordination and direct support role.
The successful applicant will progressively take responsibility for the day-to-day coordination and
operation of Meg’s care environment while remaining actively involved in providing hands-on support.
This role combines care coordination, team leadership, rostering, payroll administration, recruitment,
household and operational management, NDIS support coordination functions and direct disability
support.
Due to the highly individualised nature of Meg’s care, the successful applicant will undertake a structured
transition period to learn Meg’s individual needs, communication style, routines, equipment, household
systems and team culture.
The long-term goal of this position is for the Care Team Manager to become the primary operational
contact for Meg’s support team, allowing the Director to step back from daily operational demands while
maintaining strategic oversight.
Key Responsibilities
Direct Support & Care Delivery
The Care Team Manager will maintain a hands-on role within Meg’s care team.
Responsibilities include:
Providing direct support shifts with Meg.
Working alongside other team members to maintain familiarity with daily routines and support requirements.
Participating in approximately one rostered weekend shift per month.
Providing direct support during planned leave periods, unexpected staff shortages or operational requirements.
Maintaining competency in all aspects of Meg’s care, including personal care, positioning, communication systems, equipment use and established care routines.
Supporting new staff through practical guidance, mentoring and role modelling.
Maintaining involvement in direct care is an essential part of this position and enables the Care Team. Manager to lead from practical experience.
Care Coordination & Health Management
Coordinate Meg’s day-to-day care requirements.
Maintain oversight of health needs and support plans.
Liaise with medical professionals, hospitals, nurses and allied health providers.
Organise and coordinate medical and allied health appointments.
Follow up recommendations and ensure actions are completed.
Maintain health-related documentation and records.
Coordinate equipment reviews, servicing, repairs and replacement requirements.
Support communication between providers, family and the care team.
NDIS & Support Coordination Functions
Assist with implementation and monitoring of Meg’s NDIS plan.
Coordinate and maintain relationships with service providers.
Identify gaps in supports and assist with sourcing appropriate services.
Maintain provider communication and documentation.
Assist with NDIS reviews and preparation of relevant information.
Work collaboratively with plan management and Funds in Court representatives.
Understand the purpose and limitations of NDIS-funded supports.
Team Leadership & Workforce Management
Support the development of a skilled, consistent and reliable care team.
Assist with recruitment and interviewing.
Support staff induction and onboarding.
Coordinate team communication.
Support staff training and competency development.
Assist with addressing workplace issues.
Promote accountability, professionalism and a positive team culture.
Maintain a respectful environment where staff feel supported and valued.
Rostering & Payroll Administration
Manage and maintain staff rosters.
Coordinate leave requests and availability.
Respond to unexpected staff absences and assist with finding solutions.
Maintain effective staffing coverage.
Progressively take responsibility for fortnightly payroll processing.
Review timesheets and payroll information.
Ensure employee records are accurate and maintained.
Liaise with the Director regarding payroll, employment changes and approvals during transition.
Household & Operations Management
Coordinate household maintenance requirements.
Liaise with tradespeople and service providers.
Manage administration relating to equipment, vehicles and household operations.
Maintain records and systems to ensure continuity.
Assist with identifying improvements to systems and processes.
Support the smooth running of Meg’s home environment.
Office Administration & Systems
Manage day-to-day administrative tasks.
Maintain digital filing and record systems.
Use Microsoft Office and relevant software applications.
Identify and implement practical technology solutions.
Maintain employment and compliance documentation.
Support efficient information management.
Travel & Community Participation
The role may include:
Attending appointments with Meg.
Supporting community participation.
Assisting with planned travel.
Participating in occasional overnight stays or holidays with Meg and the support team.
Providing coordination and leadership during these periods.
Developmental Pathway
The role includes a structured transition period.
First 6 Months – Learning & Integration
The focus will be on:
Learning Meg’s care needs and routines.
Understanding household systems.
Building relationships with Meg, family and the care team.
Learning rostering, payroll and administrative processes.
Developing confidence in coordination responsibilities.
Ongoing Development
Responsibilities will progressively increase as competency and confidence are demonstrated. Salary progression will be based on demonstrated capability, independence and successful transition into the full responsibilities of the position.
Essential Criteria
Experience working within disability, health, community services, care coordination or a related
field.
Demonstrated experience working with people and coordinating services.
Strong organisational and administrative skills.
Excellent communication and relationship-building skills.
Ability to work independently and manage competing priorities.
Strong computer literacy.
Ability and willingness to undertake direct disability support work.
Desirable Criteria
Registered Nurse qualification.
Certificate IV or higher in Disability, Community Services or related field.
Experience supporting people with complex physical disability.
Experience within a self-managed NDIS environment.
Experience with rostering and payroll systems.
Experience leading small teams.
Experience with AAC, PEG feeding, complex health needs or rehabilitation environments.
