Key Responsibilities:

• Develop and implement personalized care plans for clients in home care settings.
• Conduct assessments and evaluations to understand client needs.
• Collaborate with families, healthcare professionals, and support staff to ensure optimal client care.
• Monitor progress and adjust care plans as necessary.
• Maintain accurate and up-to-date client records.

Qualifications:

• Must have prior case management experience. Candidates with only Personal Care Assistant (PCA) or support worker backgrounds will not be considered as this role requires someoen with indpeth knwoledge of home care package mnagement.
