A respected, values-driven aged care organization is seeking an experienced and dedicated Clinical Care Coordinator to join their team in Portland, Victoria on a Full-time basis.

About the Role

As the Clinical Care Coordinator, you will play a vital leadership role in ensuring residents receive the highest standard of person-centred clinical care. You will be instrumental in driving quality and safety across the facility.

Key Responsibilities Include:

  • Assess resident needs comprehensively and develop tailored care plans in collaboration with families and residents.

  • Lead, mentor, and support a dedicated nursing and care team to deliver safe, high-quality care outcomes.

  • Maintain accurate clinical documentation and oversee compliance with all relevant aged care requirements.

  • Provide hands-on nursing care when needed, including wound management, medication administration, and monitoring.

  • Support infection control measures, clinical audits, and quality improvement initiatives.

  • Act as a professional role model and effective communicator with residents, families, and external health professionals.

What We Are Looking For

We are seeking a candidate who brings a strong clinical background and proven leadership skills:

  • Bachelor of Nursing and current AHPRA registration.

  • Substantial experience in residential aged care nursing.

  • Demonstrated strong clinical assessment, documentation, and leadership skills.

  • Excellent communication, interpersonal skills, and computer literacy (including clinical software).
