Community Care Coordinator

St Vincent's Hospital

About the Role
As a Care Coordinator you will manage a case load of clients and undertake comprehensive assessments, care planning, work with clients to achieve their goals and exit from the program with long term supports in place. As an Occupational Therapist you will provide high, quality, evidence based assessment, intervention, education and management, supporting clients to remain as independent as possible. Clinical service delivery is undertaken in the clients’ own home and their local community.

  • Direct clinical provision of high quality and evidence-based Occupational Therapy specific assessment, intervention, education and management.
  • Undertake holistic comprehensive assessments, collaborative care planning and facilitation of clients generated goals using motivational interviewing and self-management approaches.
  • Monitor goal achievement and health outcomes and plan for the transition and exit from the program, ensuring appropriate long term community supports are in place.
  • Provide guidance support and direction to Allied Health Assistants

Classifications for this position will range from classification range Grade 2 VF6 - VF9 ($90,235 to $103,724 per annum).

Your Contribution

  • Current registration with AHPRA as an Occupational Therapist
  • Minimum 2 years’ experience in relevant acute/community setting
  • Current Victorian Driver’s License
  • Experience as an Occupational Therapist working with clients with multiple health issues, comorbidities and complex care needs that impact safety, function, independence and occupational performance.
  • Experience delivering services in the community, addressing the needs of clients living with chronic diseases, aged related complex health needs, and psychosocial conditions.
  • Current National criminal history check, or willing to obtain

About Complex Care and Chronic Disease

This position will be part of the HIP Complex Care and Chronic Disease Service (CC/CD) under the Health Independence Program. The key aim of the CC/CD Service is to improve health outcomes for people who either frequently present, or who are at risk of presenting to hospital, because of complex medical and or social issues. CC/CD currently consists of a multidisciplinary team based at SVHM Fitzroy. Clinical service delivery is undertaken in the clients’ own home and their local community.
