The service
- Acute Intervention / Crisis & Assessment Team
- Community-based acute care, hospital avoidance, and step-up/step-down support
- Close integration with:
- Adult inpatient services
- Emergency departments
- Community mental health teams
- Primary care
- Consultant responsibility for acute community presentations
- Rapid assessment, diagnosis, and treatment planning
- Clinical leadership within a senior MDT
- Support for crisis resolution and hospital diversion
- Supervision and mentoring of junior doctors
- Opportunity to contribute to service development and models of care
