District Nurse - regional Victoria

We are seeking a dedicated and autonomous District Nurse (Registered Nurse) to join our community health team in regional Victoria. In this role, you will step out of the rigid hospital ward and deliver vital, high-quality clinical care directly to patients in their own homes and community settings. From complex wound management to palliative care, your clinical expertise will help local residents maintain their independence and health.

Please Note: This position is located in regional Victoria and is not commutable from Melbourne. A relocation to our beautiful regional community is required. To help you make the move, we are offering comprehensive relocation and accommodation support.

Key Responsibilities

  • Deliver comprehensive, evidence-based nursing care to patients in their homes, including wound care, medication administration, and chronic disease management.

  • Conduct holistic patient assessments, design individual care plans, and monitor patient health trajectories.

  • Provide compassionate palliative and end-of-life care, ensuring dignity and comfort for patients and support for their families.

  • Collaborate closely with local GPs, allied health professionals, and regional hospital networks.

  • Maintain accurate, compliant clinical documentation and champion patient safety in the community.

Key Requirements

  • Registration: Current, unrestricted registration as a Registered Nurse with AHPRA.

  • Experience: Prior experience in community nursing, district nursing, aged care, or acute medical/surgical wards is highly regarded.

  • Independence: Strong clinical assessment skills and the ability to work confidently and safely with minimal direct supervision.

  • Communication: Exceptional interpersonal skills to build trust with diverse patients, families, and local healthcare providers.

  • Logistics: A valid Australian driver’s licence is essential (a fleet vehicle is provided for patient visits).
