About Department
This position is part of the Health Independence Program (HIP). The intent of HIP is to deliver improved outcomes for clients and to support hospital demand management and flow, through delivering integrated and coordinated care to clients across the hospital and community interface, reducing duplication and fragmentation of services and ensuring client centred care planning. The Young Adult Complex Disability service (YACDS) is a state-wide multidisciplinary service for adults aged under 40 with a history of developmental disability, resulting in complex medical needs and physical disability. The key aim of the YACDS support young people and their family transition from paediatric services to adult healthcare services, and improve their health outcomes.

About the Role
You will provide high quality occupational therapy services to the clients of the Young Adult Complex Disability Service. You will work in collaboration with the multidisciplinary YACDS team and other SVHM clinical team’s, as well as external organisations such as the NDIS. You will deliver clinical care within varied settings including; outpatients (currently Fitzroy and Kew locations), in the clients’ own home and their local community.

Classifications for this position will range from Occupational Therapist Grade 2 VF6 – VF9 ($90,235.60 - $103,724.40) per annum, pro rata for part time.

Duties

• Delivery of accountable, client focused, high quality occupational therapy services, such as; assessment, treatment, evaluation, referral management, reviews, and carer support to all clients within your caseload
• Drive best practice based on evidence and demonstrated outcomes
• Work in collaboration with clients, team members, referring professionals, community professional and other key stakeholders
• Contribute to service and stream development
• Provide supervision, guidance and support to allied health assistants and undergraduate allied health students
• Participate in continuous improvement activities within the YACDS

Qualifications, Skills and Experience

• Bachelor of Occupational Therapy
• Minimum of four (4) years’ post-graduate experience
• Eligibility for membership of relevant discipline association
• Knowledge and experience regarding the delivery of services within the disability sector
• Highly developed interpersonal skills that incorporate the qualities of excellent communication and leadership
