Our client is seeking a Health Improvement Practitioner to join their general practice team. You will deliver brief, consultation-based services focused on evidence-based behavioural interventions for both mental and physical health conditions. This role is central to supporting an integrated ‘Model of Care,’ promoting seamless collaboration between general practice, community NGOs, and specialist services. Your care, empathy, and culturally responsive approach will help guide patients toward holistic well-being.

Key Responsibilities
- Provide evidence-based behavioural interventions in individual, group, and whānau settings
- Collaborate with the GP team to support consultation-based services
- Guide clients through available health services to improve overall well-being
- Promote prevention, acute care, and chronic disease management through tailored interventions
- Engage in continuous professional development
Skills and Experience
- Full Registration with the Social Worker Registration Board, Occupational Therapy Board, or New Zealand Association of Counsellors
- A current practising certificate as a registered health professional (psychologist, nurse, occupational therapist, social worker)
- Clinical experience in delivering mental health and psychological interventions
- Familiarity with Te Reo me ōna tikanga and experience working in diverse Māori settings
- Strong communication skills, with the ability to work with Māori, Pasifika, and youth communities
- Excellent relationship management and organisational skills
