Nurse Practitioner | Palliative Care

St Vincent's Hospital

About the Role

The Nurse Practitioner contributes to the successful operation of Palliative Care Consultancy Service (PCCS). The role will be responsible for assessing and reviewing patient and family care needs, symptom management, future care planning, liaising with community palliative care services in Victoria and other stakeholders, contributing to discharge planning, and educating and mentoring clinicians throughout SVHM.



The NP will demonstrate advanced level expertise in the provision of end of life care and palliative care management within the boundaries of the Palliative Care Nurse Practitioner (PCNP) scope of practice, legal and ethical parameters, and with a focus on delivering excellence and best practice. The NP will also contribute to service development and ensuring that the service provides high quality, evidence-based clinical services that leads to improved care of patients and their families.



The classification for this position is NO2 ($134,934. base salary per annum pro rata).



Duties

  • Provide clinical leadership
  • Demonstrate advanced nursing skills in the assessment, planning, implementation and evaluation of clinical services to patients with life limiting illness and their carers.
  • Develops and maintain own clinical development and competence, in line with best practice standards and NMBA regulations, particularly in the areas of:
    • Advanced health assessment and diagnostic skills
    • Advanced clinical decision making
    • Pharmacologial intervention and prescribing
    • Radiology and Pathology investigation requests and interpretation of results
  • Identifies potential adverse outcomes and implements proactive strategies to achieve risk minimisation
  • Actively engages community/public to inform interventions, referrals and coordination of care
  • Actively promotes the NP role and advanced nursing practice through key activities, for example (but not limited to): presentations at local, health services and with key stakeholders and partners, leading departmental meetings, establishing and contributing to local working groups and/or special interest groups
  • The NP is required to be a role model to all staff in areas of clinical practice
  • The NP will engage in the education of the palliative care team, clinical areas of SVHM and the wider community about all aspects of palliative care.
  • Leads and contributes to quality improvement and best practice activities that evaluate current practices in the clinical area
  • Supports development and participates in development and data entry of key indicators for the PCCS, to enable evaluation and benchmarking of the PCCS against similar services.
  • Presents and publishes at/in appropriate professional conferences and journals




Your contribution

  • Registered Nurse Practitioner with the Nursing and Midwifery Board of Australia and AHPRA registered.
  • Demonstrated experience practicing at an advanced clinical practice level in the area of palliative care
  • Demonstrated ability to work with a multidisciplinary team and work both autonomously and collaboratively
  • Demonstrated ability to be self-motivated and innovative
  • Demonstrated practice of critical reflection
  • Demonstrated ability to make appropriate clinical decisions which consider cultural and diversity sensitivity and alignment to meet the goals and expectation of the patient and family.
  • Excellent communication skills and ability to share knowledge with other clinicians across multiple disciplines/settings
  • Demonstrated interpersonal and leadership skills including problem solving, conflict resolution, negotiation and teaching
  • Demonstrated ability to achieve change.
  • Knowledge of workplace statutory and regulatory requirements and their impact on nursing care delivery
  • Demonstrated ability to implement evidence-based care to improve delivery of care and patient outcomes
  • Demonstrated ability to facilitate and coordinate nursing education and research projects
  • Demonstrated commitment to ongoing learning and professional development

About Palliative Care Services



Palliative Care at SVHM is delivered by coordinated medical, nursing and allied health services that are provided, where possible, in the environment of the person's choice. Currently, inpatient palliative care is provided at Caritas Christi in Kew with 16 inpatient beds; a Palliative Care Consultancy Service in Fitzroy; and aligned with the Centre for Palliative Care.



The Palliative Care Consultancy Service (PCCS) is based at St Vincent’s Hospital Fitzroy and provides specialist palliative care consultation to clinicians, patients and families within SVHM, St George’s Hospital, St Vincent’s Private Hospital, and other affiliate organisations.
