• Based at St Georges Hospital Kew

• Fixed term opportunity: 24 hours per week from June to July 2026, then 32 hours per week until 24th Dec 2026.


About the Role

An opportunity to join St Vincent’s Hospital Melbourne in a Grade 2 Occupational Therapist role within the Transition Care Team. The successful applicant will provide high quality occupational therapy services; supporting assessment, intervention and discharge planning for clients on TCP.
You will work within a highly skilled multidisciplinary team to deliver outcomes that are determined by client care plans and goals. The role is responsible for the supervision of students and allied health assistants.
Applications from current grade two occupational therapists or suitably experienced grade one occupational therapists will be considered. Relevant postgraduate qualifications would also be highly regarded.

Duties:
· Provide high quality and optimal occupational therapy assessment and clinical management of Transition Care Program clients.
· Plan, implement and monitor appropriate treatment programs from established client goals using best practice principles and evidence-based practice.
· Provide carer/family support to assist in the management of Transition Care Program clients at home.

Classifications for this position will be Occupational Therapist Grade 2 VF6 - VF9 ($90,235 - $103,724 per annum, pro rata for part time).

Qualifications, Skills and Experience
· Tertiary Occupational Therapy qualification.
· Eligibility for membership of the Occupational Therapy professional association.
· Minimum of 5 years’ experience in a health care setting
· Sound knowledge of Occupational Therapy in aged care.

  • Current National criminal history check, or willing to obtain

    About Transition Care
    The Transition Care Program has a multidisciplinary team that provides case management and allied health therapy to 26 Bed based clients and 45 clients based in the surrounding community.
