About the Role
An exciting opportunity exists for a Grade 2 Occupational Therapist to join the Health Independence Programs (HIP) Community Rehabilitation team. In this role, you will deliver high-quality, person-centered, goal-oriented rehabilitation services in both home and center-based settings. Working within a supportive multidisciplinary team, you will help clients achieve their rehabilitation goals and maximise their independence through evidence-based assessment, intervention and care planning.
Classification: Occupational Therapist Grade 2 ($90,235 to $103,724 per annum).
Your Contribution
- Bachelor of Occupational Therapy or equivalent qualification.
- Current registration with the Australian Health Practitioner Regulation Agency (AHPRA).
- Minimum of four years' postgraduate experience in rehabilitation and/or community-based services.
- Demonstrated clinical experience and expertise in community rehabilitation.
- Strong organisational, planning and problem-solving skills.
- Highly developed interpersonal, written and verbal communication skills.
- Demonstrated commitment to continuous quality improvement and professional development.
- Understanding of, and commitment to, the principles of the Health Independence Program (HIP), including delivering individualised, goal-centred and time-limited episodes of care that optimise client outcomes.
- Commitment to the values and healthcare philosophy of St Vincent's.
- Current Victorian Driver Licence.

About the Department
This position sits within the Health Independence Programs (HIP), which aims to improve client outcomes and support hospital demand management through the delivery of integrated, coordinated care across the hospital and community interface. HIP focuses on reducing duplication and fragmentation of services while ensuring person-centred care planning.
St Vincent's Hospital Melbourne HIP operates across several integrated hub sites, including Fitzroy, Kew and Fairfield, and is delivered in partnership with North Richmond Community Health. This position is part of the Community Rehabilitation team, with services currently delivered from our Kew and Fairfield centres.
The primary aim of Community Rehabilitation is to support people recovering from traumatic injury or illness, as well as those living with frailty or chronic illness, to regain optimal function and maximise their independence. Our team provides a supportive, collaborative and dynamic environment with a strong focus on person-centred care.
