Physiotherapist / Care Coordinator

About the Role

Physiotherapist: As a Physiotherapist, you will provide assessment, intervention and management to achieve optional functional recovery, encourage self-management and promote empowerment for clients to improve their health outcomes, wellbeing and quality of life. You will be required to work with clients in the community with multiple health issues, comorbidities and complex care needs that impact their wellbeing.

Care Coordinator: As a Care Coordinator you will provide person-centred care coordination for high-risk clients living with complex aged, health and psychosocial care needs. You will conduct comprehensive holistic health assessments, coordinating all aspects of health and social care needs, monitoring achievement of client goals, facilitating intervention from other health care professionals and ensuring appropriate linkages with hospital and community services.

  • Direct clinical provision of high quality and evidence-based Physiotherapy specific assessment, intervention, education and management.

  • Manage a diverse complex caseload of aged care, falls, neurological conditions, vestibular conditions, chronic pain conditions, chronic respiratory issues, cardiac conditions, diabetes and other comorbidities.

  • Undertake holistic comprehensive assessments and care planning in collaboration with the client and/or carers using self-management approaches.

  • Monitor health outcomes and plan for the transition and exit from the program, ensuring appropriate long term community supports are in place on discharge.

  • Work with and alongside an Allied Health Assistant providing the necessary handover, guidance and support.

Classifications for this position will range from VB1 – VB4 ($86,730 - $99,699 per annum).

Your Contribution

  • Current registration with AHPRA as a Physiotherapist
  • Victorian Driver’s
  • Broad Physiotherapy assessment and treatment skills, including acute musculoskeletal, cardio respiratory and acute neurological.
  • Experience delivering services in the community.
  • Excellent organisation and time management skills.
  • Current National criminal history check, or willing to obtain

About Complex Care
Complex Care is part of the Health Independence Program. The key aim of the Complex Care service is to improve health outcomes to people who either frequently present, or are at risk of presenting to hospital, because of complex medical and or social issues. Complex Care currently consists of multidisciplinary teams based at Fitzroy, and at North Richmond Community Health. Clinical service delivery is undertaken in the clients’ own home and their local community.

This position is based at North Richmond Community Health.
