As a Psychogeriatric Case Manager in an Aged Care Unit, you will provide specialised support, including early intervention, thorough assessments, and effective case management. As well as educating and guiding service providers to maintain high care standards.

**Key Responsibilities**

  • Perform evidence-based assessments and create personalised case management plans
  • Psycho-social interventions and counselling in acute, community, and residential care
  • Work with internal teams and external partners for smooth continuity of care.
  • Aid clients and families with discharge planning
  • Engage in networking and negotiation to enhance resources and support for clients and carers
  • Perform ACAT assessments on patients independently

**Essential Requirements**

  • Qualification in nursing, allied health, or related field
  • Registered with AHPRA (Australia Health Practitioner Regulation Agency)
  • Valid NT Police Check
  • Valid NT Working with Children Check
  • ACAT Certified
  • Have 2+ years' experience working with older people dealing with dementia and mental health issues
  • Exhibit effective communication, interpersonal skills and show cultural awareness
  • Willingness to travel to remote regions
  • Hold NT Driver’s C Class Licence or ability to obtain one
