Registered Nurse / Care Coordinator

About the Role

An exciting opportunity exists for an experienced and compassionate Care Coordinator to join St Vincent's Hospital Melbourne's Complex Care and Chronic Disease team within the Health Independence Program (HIP). In this rewarding role, you will provide person-centred care coordination for clients with complex aged care and healthcare needs, supporting them to improve their health, wellbeing and independence. Working as part of a multidisciplinary team, you will deliver discipline-specific clinical care while coordinating services to ensure clients receive the right support at the right time. This community-based role is primarily focused on supporting older adults living with chronic disease and complex health needs in their homes and local community.

Key Responsibilities

• Conduct comprehensive, holistic assessments considering clients' health, functional, psychosocial, cultural, environmental, safety and financial needs.
• Develop, implement and review person-centered care plans that align with clients' goals.
• Coordinate multidisciplinary and specialist services to address complex client needs.
• Monitor client progress, evaluate outcomes and support successful transition from the program with appropriate long-term community supports.
• Deliver high-quality, evidence-based discipline-specific assessment, intervention, education and clinical management.

Classification: YY4 ($121,695.60 per annum).

Your Contribution

To be successful in this role, you will bring:

• Current AHPRA registration.
• Current Victorian Driver Licence.
• Experience working with older adults living with chronic disease and complex health and psychosocial needs.
• Excellent assessment, care coordination and communication skills.
• Current National Criminal History Check (or willingness to obtain).

About Complex Care & Chronic Disease

The Complex Care and Chronic Disease team forms part of St Vincent's Health Independence Program (HIP), providing multidisciplinary care to people with chronic and complex healthcare needs who frequently present, or are at risk of presenting, to hospital. Our goal is to improve health outcomes by delivering coordinated, person-centered care in clients' homes and local communities. The team is based across St Vincent's Hospital Melbourne (Fitzroy) and North Richmond Community Health, with this position primarily located at our Fitzroy campus.
