Renal Vascular Access Coordinator

About the Role
The Renal Vascular Access Clinical Nurse Consultant (CNC) is a registered nurse who has acquired the expert knowledge base, complex decision making skills and clinical competencies for advanced practice. The Renal Vascular Access CNC is an integral member of the health care team who practices in collaboration with the Nurse Practitioner, Nursing team, Nephrologists, Vascular Surgeons, Interventional Radiologists, Anaesthetists and other health professionals to assess and manage patient care within their clinical context using nursing knowledge and skills

Classifications for this position will be classification ZJ4 ($110,333.60 per annum pro rata).

You will also:

  • Act as the pivotal point of communication and co-ordination for patients in the CKD program for referring consultants, dialysis nursing and allied team, patients and their families.
  • Monitor clinical and pathology parameters, with respect to developing and finalising CKD treatment plan as determined by CKD clinicians.
  • Provide individual, group and inpatient education in relation to chronic kidney disease, renal replacement therapy, conservative/supportive treatments, and maintenance dialysis utilising principles of adult learning and education.
  • Promote strategic treatment of ESRF, utilizing all treatment options including transplant and home dialysis as ‘first line’ treatment.
  • Partners with patients in health promotion, including referral to dietitian, diabetes nurse educators, social work and local/regional healthcare services.

Your Contribution

  • Degree in general Nursing which meets the registration requirements of the Nursing and Midwifery Board of Australia
  • Current Nursing Registration with AHPRA
  • Post Grad Certificate in Renal Nursing
  • Significant postgraduate nursing experience in Nephrology
  • Comprehensive knowledge and skill in managing CKD pathways
  • Current National criminal history check, or willing to obtain

About St Vincent's Hospital Renal Service
St Vincent’s Hospital Renal Service provides care for patients across the entire spectrum of renal disease, from the ambulatory care of those with CKD to meeting the dialysis and transplant needs of those with ESKD. SVHM is known as a hub provider of renal services supporting a number of regional and rural satellite as well as in-centre renal dialysis facilities.

Our dialysis service comprises haemodialysis (acute and chronic), home dialysis (home haemodialysis and peritoneal dialysis) with the service currently providing medical support for 330 patients undergoing maintenance dialysis and 350 patients with CKD stage 4/5 (all approaching ESKD and dialysis) state wide.
