Health Independence Program - Community Rehabilitation

About the Role
An exciting opportunity currently exists for a Grade 2 Social Worker to join the Community Rehabilitation team on a fixed term part time basis. You will work collaboratively with clients and carers to support and empower them to remain as independent as they can and remain socially active within their communities.
- Provide time-limited, goal–orientated care for people who have a physical disability, are frail, chronically ill or recovering from traumatic injury or illness to regain and /or maintain optimal function
- Link clients and carers in with appropriate services and community supports to enhance their outcomes.
- Deliver services in the Community Rehabilitation Centre, the community setting and the client's home.
Classifications for this position will range from Social Worker Grade 2 SC21 - SC24 ($90,235 - $103,724 per annum pro rata).
Your Contribution
- Bachelor of Social Work
- Relevant experience, preferably in the rehabilitation setting
- Knowledge of relevant community resources
- Demonstrated ability to work effectively in an interdisciplinary team
- Current Victorian driver’s licence
- A valid Nationally Coordinated Criminal Hisotry Check, or willing to obtain

About Health Independence Program (HIP)
HIP includes Complex Care Services, Community Rehabilitation Services and specialist clinics including the Continence service, Cognitive, dementia and Memory service (CDAMS), Falls and Balance clinic, Barbara Walker Centre for Pain Management (BWCPM), Polio Services Victoria (PSV), and the Young Adult Complex Disability Service (YACDS).
HIP aims to delivers improved outcomes for clients and to support hospital demand management and flow, through delivering integrated and coordinated care to clients across the hospital and community interface, reducing duplication and fragmentation of services and ensuring client centred care planning.
The Community Rehabilitation services provide time-limited, goal–orientated care for people who have a physical disability, are frail, chronically ill or recovering from traumatic injury or illness to regain and /or maintain optimal function.
Community Rehabilitation services include Community rehabilitation, cardiopulmonary rehabilitation and Rehabilitation in the Home.
