About the Role
The Young Adult Complex Disability service (YACDS) is a state-wide multidisciplinary service for adults aged under 40 with a history of developmental disability, such as cerebral palsy, or other medical conditions resulting in chronic physical disability. The key aim of the YACDS is to help these young people and their family transition from paediatric services to adult healthcare services, and improve their health outcomes.

  • Delivery of accountable client focused, high quality referral triage, assessment, care planning, referral management, reviews, counselling and carer support within caseload
  • Collaborative work with the multidisciplinary team members, referring professionals, community professionals and other key stakeholders within a flexible, multidisciplinary structure
  • Communicate client issues effectively with other team members and actively contribute to multidisciplinary team meetings


Classifications for this position will range from SC31 to SC34 ($102,091 - $116,646 per annum, pro rata).

Your Contribution

  • Bachelor of Social Work
  • Minimum of seven (7) years’ post-graduate experience
  • Eligibility for membership of relevant discipline association
  • Post graduate qualifications highly desirable
  • Current National criminal history check, or willing to obtain
  • Working with Children check, or willing to obtain

About Health Independence Program (HIP)

The intent of HIP is to deliver improved outcomes for clients and to support hospital demand management and flow, through delivering integrated and coordinated care to clients across the hospital and community interface, reducing duplication and fragmentation of services and ensuring client-centred care planning. SVHM HIP consists of integrated hub sites across several locations including in Fitzroy, Kew, North Richmond and Fairfield.



This position will be part of the HIP Disability stream, initially based at the Fitzroy site.
