About the Role
This position is 0.5 FTE Team Leader and 0.5 Senior Clinician, for the Mobile Support and Treatment Service.
You are a Senior Mental Health Clinician who brings extensive experience in providing support to consumers from an intensive home, and community based, Recovery orientated practice. Your main roles will consist of:
- Supporting the MST manager in overseeing daily operations
- Develop the roster in line with SVHM roster policy
- Support chairing of the clinical review at times when the manager is absent
- Attend weekly intake meeting to review consumers suitable for case management with MST
- Support monthly line supervision in alignment with the EBA, CWMS requirements
- Offer coaching and mentorship to junior case managers, especially within Mobile Support and Treatment Services, catering to consumers with complex needs
- Attend senior clinical leadership meetings to support the effective functioning of services, particularly within the Hawthorn Clinic.
- Foster strong therapeutic alliances with consumers and actively involve them, along with their family/carers/supporters, in comprehensive care planning and evaluation processes.
- Develop strong partnerships across the mental health program with inpatient, ED
- Conduct thorough assessments, deliver targeted interventions, and provide treatment for individuals with intricate mental health needs, aligning practices with local policies and strategic directions.
- Contribute significantly to primary, secondary, and tertiary consultation services, actively engaging in community development, education, and establishing effective liaisons with other supportive agencies.
- Ensure strict compliance with activity-based funding requirements, diligently recording daily contacts, phase of care, outcome measures, and any ICD10 diagnoses
Classifications for this position will range from:
Senior Clinician:
- OT/SW Grade 4 HR9 - HR12 or HR25 - HR28 ($107,260 - $122,548 per annum pro rata)
- RPN Grade 4 NP75 - NP77 ($121,274 - $130,603 per annum pro rata)
Team Leader:
- RPN Grade 5 NP51 ($136,822 per annum pro rata)
- OT/SW Grade 5 AJ11 - AJ14 or AJ57 - AJ60 ($129,859 - $143,098 per annum pro rata)
Your Contribution
- A minimum of 7 years post qualification clinical experience desired within Mental Health
- Occupational Therapists: registration with the Australian Health Practitioner Regulation Agency (AHPRA) Completion of undergraduate Degree in Occupational Therapy OR Masters of Clinical Practice in Occupational Therapy.
- Social Workers – An approved degree in Social Work (by the AASW) and eligibility for full membership of the Australian Association of Social Workers. International Social Workers will need to be able to provide evidence of their assessment of international qualifications from the AASW (inclusive of the reference number).
- Nursing: Completion of post graduate diploma or a specialist undergraduate mental health nursing program or, a specialist post basic course of training which led to previous registration as a division 3 nurse. Registrations with AHPRA.
- A valid Nationally Coordinated Criminal History Check, or willing to obtain
