About the Role
An exciting opportunity exists for a Grade 3 Social Worker to join the Transition Care Program. The successful applicant will be responsible for providing high quality, evidence-based Case Management support to aged care clients in Transition Care and leading day-to-day operational functions within the program. The role is based at St George’s Hospital in Kew and works as part of a close-knit, multi-disciplinary team. Working at a Grade 3 level, the Team Leader - Operations will hold leadership responsibilities, including supporting patient flow for the Transition Care program at St Vincent’s. This will be balanced against a clinical caseload of bed-based clients requiring Case Management input and complex discharge planning (including placement in Residential Aged Care).
This is a rare opportunity for an enthusiastic Social Worker with aged care experience to join a cohesive and progressive integrated team, committed to excellence in clinical practice, research and professional development. The incumbent must hold, or be currently eligible for, membership with the Australian Association of Social Workers (AASW) and relevant postgraduate qualifications will be highly regarded.
Classifications for this position will be: Social Worker Grade 3 SC31 – SC34 $104,135 - $118,981 per annum, pro rata for part time).
Duties include
- Provision of high quality, evidence-based, patient centred care.
- Collaborative care planning with clients, carers and/or families in the context of their health, functional, psychosocial, cultural, environmental and economic needs.
- Leadership support with day-to-day operational functions including: program intake, occupancy management, managing staff performance and leading team meetings.
- Collaboration with the multidisciplinary care team, including the provision of secondary consultation to staff in area of expertise.
- Drive quality and continuous improvement initiatives, team processes and service development activities
- Application of case management principles and practice frameworks including strengths based approaches.
Your Contribution
- Significant Social Work experience in Aged Care and awareness of the policy environment, including current knowledge of the Residential Aged Care sector and Victorian Civil Administrative Tribunal processes
- Membership of, or eligibility for, AASW Membership
- Demonstrated ability to lead and support a multidisciplinary team
- Proven ability to work collaboratively with a range of stakeholders, including clients, families, external agencies and other health professionals
- Highly developed interpersonal skills that incorporate the qualities of excellent communication, negotiation, organisation, planning and problem solving
- Knowledge of and commitment to continuous quality improvement processes
- Superior written and verbal communication skills
- Demonstrated knowledge of the application of case management practice frameworks and principles, in the context of the current health care environment.
- Well-developed teaching and education skills
- Evidence of commitment to professional development in relevant areas

Transition Care Program
The Transition Care Program (TCP) is jointly funded by the Commonwealth and State and Territories. St Vincent’s TCP is resourced to follow the client through the service system and avoid unnecessary extension of hospital admissions and/or premature admission to residential care.
The intent of the Transition Care Program is to provide short-term support and active management for older people at the interface of the acute/subacute and residential aged care sectors.
The target group for Transition Care services at St Vincent’s are:
- Older people exiting from hospital who, via participation in the program, achieve further improvements to their physical, cognitive and psychosocial functioning and an improved capacity for living independently; and/or
- Older people exiting from hospital for whom the focus will be on actively maintaining their functioning while assisting them and their family/carers make appropriate long term care arrangements.
The Transition Care Program is located at the St Georges Health Service in Kew, but services all of the St. Vincent’s Hospital Melbourne catchment.
